What is concierge health and wellness?

We are an integrative health practice focussed on personalized, holistic health and wellness services. Our concierge model offers health and wellness services that are traditionally offered in a clinic setting in the comfort of our patients' home or business.

What is Functional Medicine?

 

Functional Medicine is a systems biology–based approach that focuses on identifying and addressing the root cause of disease. Each symptom or differential diagnosis may be one of many contributing to an individual’s illness.

 

Where can I find pricing information?

Since we offer a variety of services that can change from time to time we do not list prices on our website. If you want to inquire about the cost of a service please complete our contact form and someone from our office will provide you with the information.

 

Do you accept insurance?

 

We are a concierge mobile practice and provide services for individuals who desire personalized care and convenience. Our business model does not fit with the health insurance model of care. The excellent care and convenience you will receive from our practice will more than exceed the costs you pay for the services. We can provide a superbill for insured patients to submit to their insurance providers to request reimbursement however we cannot guarantee reimbursement. We also accept HSA and FSA debit and credit cards. Other forms of payment include, credit card, PayPal, and cash. No personal checks.

Should I book a telehealth visit or a house call?

Many conditions can be treated via telehealth. If you require a diagnostic test, screening test, procedure, or a physical examination of any part of your body than can't be evaluated through a virtual visit, you should have a house call.

What is the cost for a House Call?

The cost of a one time medical house call will depend on a number of factors which include your location, how many people in the home are needing services, and the type of health services being provided. Please contact us to discuss your needs and to receive a quote for services.

How far will your company travel for a house call?

We primarily serve Hendersonville, Gallatin, and Goodlettsville for visits requiring a house call however we will travel outside those areas for an additional fee. Please contact us so we can discuss the costs for your location.

Do I need to live in a certain area for a telehealth or virtual visit?

Telehealth and virtual visits are available for anyone residing in the following states the Tennessee, Washington, Florida, Arizona, Massachusetts, and Nevada . We do not bill insurance for these visits and cannot currently see patients who have Medicare. 

Do you offer COVID-19 testing?

Yes! We offer rapid antigen Covid testing, rapid antibody testing, and PCR send out testing through Quest diagnostics, in Tennessee only. These tests are performed in the home or business setting. Group discounts available for more than 1 individual testing in the same location on the same day.

What happens if I pay for a telehealth visit, but need to be seen in person?

If you schedule a house call to evaluate the same problem you were treated for during your telehealth visit, the telehealth visit fee can be applied towards the cost of your house call if it is scheduled within 14 days from your initial visit.

Do you offer the COVID Vaccine?

We do not offer the COVID vaccine at this time. 

I have Medicare, can I be seen as a patient? 

Due to federal laws, JCHW is unable to see any Medicare beneficiaries for primary care services. Please check back in the future for updates to this policy. We can however see Medicare patients for non-primary care services such as mobile testing (with a physician order), IV therapy and Functional Medicine.

 

I have a question, how do I talk to someone?

 

Due to our provider being out on house calls throughout the day we have a limited availability for answering live calls. The best way to reach us and receive a call back are to send a text message to 615-239-7270, email us as info@jonesconciergehealth.com, or submit a contact form and someone will get back to you as early as possible.

I want to schedule an appointment for today but I don't see availability. What should I do?

I'm pregnant, can I use your concierge health services?

JCHW can establish your pregnancy diagnosis and begin initial prenatal care. Once your pregnancy has been confirmed with appropriate testing it is important that you establish with a midwife or OBGYN to monitor you throughout the duration of your pregnancy. JCHW can continue to manage any non-pregnancy related health concerns and for wellness services. 

Is JCHW the same as urgent care?

JCHW can treat many of the same minor illnesses and injuries that can be treated by urgent care, however we are limited to what can be evaluated in the home environment or via telehealth. Please see our list of common conditions we usually treat on our homepage. You can contact us to inquire about whether we are able to treat your condition prior to scheduling a visit.

What happens if I need bloodwork, x-rays, or other tests?

JCHW can perform point of care testing and order diagnostics to be performed at an outside facility.

What types of tests can be performed during a house call visit?

We can test for strep, flu, pregnancy, UTIs and RSV.. We can also perform screenings for hypothyroidism, anemia, h. pylori, cholesterol, blood sugar, and hemoglobin A1C. These are all rapid tests with results provided in 10 minutes or less. We can also draw blood and perform cultures that can be sent out to a lab.

What is your privacy policy?

JCHW uses HIPAA compliant electronic medical record systems to store all protected health information. We do not share any information about our patients to anyone without prior written consent.

How do I request a copy of my medical records?

You will have access to our patient portal that contains all of your medical records which can be viewed 24/7. 

Will my provider be a doctor or a nurse practitioner?

Dr. Jones is a Certified Family Nurse Practitioner who holds a Doctorate degree in Advanced Practice Nursing. Dr. Jones is not a physician (medical doctor). JCHW provides health services that are within the legal scope of practice for nurse practitioners to provide. JCHW also has a collaborative practice agreement with a licensed physician who supervises the practice.
