Do I have to be a health plan member to receive health services?

No. Members do receive priority with scheduling however you do not have to be a member to receive our health services. We offer both concierge health plans and pay per visit options. 

Why should I become a health plan member?

As a concierge health member you will have the luxury of a personal clinician to care for you and your family whenever you need it. Unlike traditional healthcare, customer service is top priority. Health plan members enjoy unlimited telehealth visits, unlimited house calls, priority scheduling, and no additional fees for screenings, tests, or procedures. Health plan members also get Dr. Jones's personal cell number and ability to contact 24-hours a day 7 days a week. Members can lower their annual healthcare costs with a concierge health plan combined with lower premium high deductible health plans to cover catastrophic or high cost hospitalizations, surgeries, etc. Finally you can get a worry free and high quality healthcare experience with affordable and predictable pricing without ever getting surprise medical bills again.

I just need one visit. Should I do a telehealth visit or a house call?

Many conditions can be treated via telehealth. If you require a diagnostic test, screening test, procedure, or a physical examination of any part of your body than can't be evaluated through a virtual visit, you should have a house call.

What is the cost for a House Call?

The cost of a one time medical house call will depend on a number of factors which include your location, how many people in the home are needing services, and the type of health services being provided. Please contact us to discuss your needs and to receive a quote for services.

How far will your company travel for a house call?

We primarily serve Hendersonville, Gallatin, and Goodlettsville for visits requiring a house call however we will travel outside those areas for an additional fee. Please contact us so we can discuss the costs for your location.

Do I need to live in a certain area for a telehealth or virtual visit?

Telehealth and virtual visits are available for anyone residing in the state the Tennessee. We do not bill insurance for these visits and cannot currently see patients who have Medicare. Telehealth visits are $49 per visit.

Do you offer COVID-19 testing?

Yes! We offer rapid Covid testing, send out testing through Quest diagnostics, and COVID 19 antibody testing.

What happens if I pay for a telehealth visit, but need to be seen in person?

If you schedule a house call to evaluate the same problem you were treated for during your telehealth visit, the telehealth visit fee can be applied towards the cost of your house call if it is scheduled within 14 days from your initial visit.

What are your office hours?

As a concierge practice we do not have traditional office hours, rather we work to arrange visits based on the patient's preference and provider's availability. We work hard to see patients the same day or next day when at all possible. 

Can I use my health insurance?

JCHW does not directly bill your health insurance, however you still have other options. If you are a member all health services are included in your monthly fee. This allows many of our patients to save their health insurance for major items such as hospitalizations or surgeries. Oftentimes this saves you significantly on your healthcare expenses rather than having to pay co-pays or meet a deductible every time you seek care. If you are a non-member, the cost of your services will need to be paid directly to the practice at the time of service. You will receive a superbill with all diagnosis codes and treatment rendered. Contact your insurance carrier and/or health savings plan to inquire about reimbursement. Many people also use their medical expenses as a tax write-off.

If you are not a member and complete a one time visit you can use your HSA card or pay with a credit, debit card, cash, PayPal or Venmo. We will provide a superbill receipt that you can use to request reimbursement. You will need to contact your insurance company to verify whether they will provide reimbursement and what their policies are.

I signed up for a health plan, what does the enrollment fee cover?

Enrollment fees cover the administrative costs to add you into our system and also serves as a deposit to protect the practice from potential abuse of our unlimited health services. We reward our loyal members by crediting back the enrollment fee after you have continued your membership for 6 months. 

What's covered in a concierge health plan?

A concierge health plan includes unlimited virtual visits, house calls, basic yearly lab tests, in home point of care testing, minor procedures, and no additional charges for medication injections and screenings. Members also receive 50% off of wellness services such as IV hydration and vitamin injections. Submit our contact form to receive a copy of our member services guide.

Are Functional Medicine visits included in the membership?

Functional Medicine visits are not included in our concierge membership, however members do receive discounts on these services. Contact us for more information.


How much does a concierge health plan cost?

Please visit our home page for pricing information as this will depend on how many people included and the type of membership selected.

I have Medicare, can I be seen as a patient? 

Due to federal laws, JCHW is unable to see any Medicare beneficiaries for primary care services. Please check back in the future for updates to this policy. We can however see Medicare patients for non-primary care services such as mobile testing (with a physician order), IV therapy and Functional Medicine.


I have a question, how do I talk to someone?


Due to our provider being out on house calls throughout the day we have a limited availability for answering live calls. The best way to reach us and receive a call back are to send a text message to 615-239-7270, email us as, or submit a contact form and someone will get back to you as early as possible.

I want to schedule an appointment for today but I don't see availability. What should I do?

If I am a patient of JCHW do I still need a primary care doctor?

If you already have a primary care provider, you can continue to use them or you can use JCHW for your baseline primary care needs for individuals ages 4 years and up. If a condition arises that is outside of our scope or can't be evaluated with a house call or via telehealth, or if you require services we don't offer we will refer you to a specialist for further evaluation and treatment. We do not offer certain health services that would be more appropriate for an office setting such as papsmears, prostate exams, etc.. If you choose to keep your PCP, JCHW can compliment their care, provide referrals and send records as needed. Children ages birth-3 can be treated for minor illnesses but should still see a pediatrician for well checks and developmental screenings.

I'm pregnant, can I use your concierge health services?

JCHW can establish your pregnancy diagnosis and begin initial prenatal care. Once your pregnancy has been confirmed with appropriate testing it is important that you establish with a midwife or OBGYN to monitor you throughout the duration of your pregnancy. JCHW can continue to manage any non-pregnancy related health concerns and for wellness services. 

I'm a female, do I still need a Gynecologist?

Since our health services are offered via virtual visits and house calls we recommend you maintain a relationship with a Gynecologist for your routine pap and pelvic exams. JCHW can perform breast exams in your home with your permission and order any needed breast imaging.

Does my child still need a pediatrician?

It important that your child continue to see a pediatrician for their routine well child exams from birth up through age 5. JCHW can still see your child for sick care and well care. We can collaborate with your child's pediatrician when your child needs to be seen in the home and will send records to the pediatrician after each visit. Once your child turns six years old you can decide whether you'd like to have JCHW take over all of their primary care needs or keep their pediatrician if your prefer.

If I am a patient of JCHW do I still need to carry health insurance?

It is a good idea to carry a high deductible health plan for coverage of major events such as hospitalizations, procedures, diagnostic studies, and specialty care. However routine healthcare services would be covered in your JCHW concierge health plan. Members who have a concierge plan can reduce their healthcare costs significantly.

Is JCHW the same as urgent care?

JCHW can treat many of the same minor illnesses and injuries that can be treated by urgent care, however we are limited to what can be evaluated in the home environment or via telehealth. Please see our list of common conditions we usually treat on our homepage. You can contact us to inquire about whether we are able to treat your condition prior to scheduling a visit.

What happens if I need bloodwork, x-rays, or other tests?

JCHW offers in home blood draws with most routine labs included in your health plan fee for members! Dr. Jones can send orders for x-rays, and other diagnostic studies as needed. These tests can be performed at local outpatient laboratories and imaging centers and results will be sent to Dr. Jones.

What types of tests can be performed during a house call visit?

We can test for strep, flu, pregnancy, UTIs and RSV.. We can also perform screenings for hypothyroidism, anemia, h. pylori, cholesterol, blood sugar, and hemoglobin A1C. These are all rapid tests with results provided in 10 minutes or less. We can also draw blood and perform cultures that can be sent out to a lab.

What if I need a vaccine?

JCHW offers limited vaccinations for members and can also write prescriptions for and administer vaccines that can be obtained from a pharmacy. 

What is your privacy policy?

JCHW uses HIPAA compliant electronic medical record systems to store all protected health information. We do not share any information about our patients to anyone without prior written consent.

How do I request a copy of my medical records?

You will have access to our patient portal that contains all of your medical records which can be viewed 24/7. 

Will my provider be a doctor or a nurse practitioner?

Dr. Jones is a Certified Family Nurse Practitioner who holds a Doctorate degree in Advanced Practice Nursing. Dr. Jones is not a physician (medical doctor). JCHW provides health services that are within the legal scope of practice for nurse practitioners to provide. JCHW also has a collaborative practice agreement with a licensed physician who supervises the practice.
